IMPORTANT!!! 

Your state does not require that your exam(s) be monitored. 
However:
Someone must sign the affidavit found below, stating that you are the one took the exam with no outside help by another person, this person can be a disinterested third party, manager or supervisor, no family members or persons working dirrectly for the student can sign the form.
