If you are in a monitored state you MUST fill out the monitor form and fax/email it to us before we can issue any certificates or credit. The monitor is responsible for making sure that you are the person taking the exam, and with a closed book. If you have any questions about who can be a monitor, please read the “Monitor Information” tab on the product page where you purchased your course.  You can fax you rmonitor form to 619-421-8171 or email it to kruise@kruise.com.
